An ASBrS Consensus Statement summarizes the indications and management considerations for a specific therapy or procedure. Consensus Statements reflect what ASBrS considers to be optimal practice, but may require modification based on the clinical circumstance, the physicians’ judgment, the patient’s preference, and the consideration that scientific evidence continues to evolve. This statement is not a systematic review of the literature. The statement was developed with a comprehensive review of the modern literature on this subject. The statement reflects the consensus of a panel of Research Committee members, with the approval of the Board of Directors, and is based on multiple sources from the peer‐reviewed literature.
