Course Description:

This course focuses on teaching participants advanced oncoplastic breast surgical techniques including volume replacement using chest wall perforator flap techniques (eg, Lateral Intercostal Artery Perforator Flap [LICAP], Anterior Intercostal Artery Perforator Flap [AICAP], etc), and advanced mammoplasty techniques including wise-pattern with complex glandular tissue rearrangement / significant transposition and rotation, with a focus on extended pedicles and secondary pedicle techniques. The course will also focus on oncoplastic techniques for central sector / retroareolar tumours including the Grisotti flap technique. Finally, the course will provide instruction on nipple reconstruction techniques.

Attendees will be required to review pre-recorded didactics prior to the course. On site, each attendee will be assigned to a station, led by a workshop faculty member, to perform the following procedures:

  • Mark-up for LICAP / AICAP / Mammoplasty on live models
  • Mark up for Central Sector Oncoplastic techniques on live models
  • Chest wall perforator flap using LICAP / LTAP on Mastotrainer
  • Chest wall perforator flap using AICAP on Mastotrainer
  • Therapeutic Mammoplasty with extended / secondary pedicle on Mastotrainer
  • Central Sector Oncoplastic Reduction using Grisotti flap technique on Mastotrainer
  • Nipple reconstruction techniques

Course Objectives:

At the conclusion of this course, participants should be able to:

  • Explain the role of chest wall perforator flaps in extending the boundaries of breast-conserving surgery, and critically evaluate flap choice based on defect location, resection pattern, and tissue characteristics.
  • Apply pre-operative planning principles to accurately mark up and understand the steps involved in safely performing chest wall perforator flap reconstructions, including LICAP and AICAP techniques.
  • Demonstrate understanding of central sector oncoplastic approaches, with particular emphasis on appropriate patient and case selection
  • Plan and execute central sector oncoplastic reconstructions, including the Grisotti flap, using sound reconstructive and aesthetic principles
  • Identify and justify the indications for extended and secondary pedicle techniques within therapeutic mammoplasty, integrating oncological safety with reconstructive outcomes
  • Design and perform Wise-pattern therapeutic mammoplasty procedures incorporating secondary or extended pedicles, with attention to surgical sequencing and complication avoidance
  • Describe and compare the range of nipple reconstruction techniques, appreciating their indications, limitations, and expected outcomes
